Laboratory Services
Schedule a due date if less than two weeks.
~ Please pour alginate impressions in stone.
~ Silicone impressions can be shipped and we will pour them for a fee.
~ We do not accept digital scans but we do fabricate appliances on 3-D printed models that you provide.
~ Enclose completed prescription slip (click here) with your signature and license number and include a business card for billing, shipping, and contact information.
~ We will return your finished case and enclose lab slips and a UPS return label for your next case.
~ Please check the models before shipping. Although come distortions are not detectable we will call if we notice any obvious problems before we fabricate the appliance to avoid unnecessary remakes.
~ Check 3-D models for voids and make sure you have printed all areas needed for the type of appliance you want, especially distal of the second molars, the palate if needed on the upper, and lingual vestibule on the lower.
